You probably missed that the aisle armrest "drops down" so you really have unlimited width. It also facilitates getting out while the tray table is open.

They should have, they are Shanghai Tang PJs. And IIRC they usually pass them out before takeoff, with the amenity kits. Not sure what happened.
Unlike the US airlines, they actually offered the pre-flight drinks.
I've always been offered preflight beverages up front on international flights on AA and CO. Domestically, CO always offers (and you can have just about anything in the galley) while AA is much less consistent.
Fresh squeezed orange juice, if you please.
