I don't know of a current link to this offer, but you can try 800-747-8202 or 800-613-3054. You can also sign up for SVO offers at the SPG website and wait for a call.
Do you know if the Avis credit must be used at the OGG location? Or can it be used at any Avis location in the U.S.? If it's a freely-usable certificate, then this is a pretty great deal. (Even if it isn't, it's not bad.) Basically Westin Maui for under $100/nt. Over the course of five days, you'll blow $100 on-property even if you are trying not to. When we were in Hawaii, sometimes a $9 pina colada would just jump into my hands when I wasn't expecting it. Scary...

To answer the Marriott question about points: the basic rules of Marriott Rewards exclude these types of prepaid packages from earning MR points. But...that's not to say they won't accidentally post. And to answer the next obvious question, I'm also pretty sure that they won't count as EQN's. I booked a multi-night about a year ago that wasn't prepaid - it was a Marriott.com normal booking, but it looked like a timeshare promo package - and I had to fax what seemed like half of Marriott corporate to get credit for that stupid stay. I kept running into people saying "Packages get no credit! Go away!!" Eventually, they got it right, but it wasn't easy.
