I wasn't talking so much about trading to transfer (since there is a limit on the number of miles you can receive each year, but instead about having the other person book the ticket for you. For example, last year I traded 60K AA miles in exchange for a member's VIPs; he applied the VIPs to my reservation and I booked the ticket for his mom. There is a certain degree of trust, but I think that if you talk to most FTers who trade, they feel pretty confident.
