Questions about 1,000 mile Paper Upgrade Certs.

I had a couple questions about the 1,000 mile upgrade certs., particularly the Visa ones.

1) What are the rules as far as the name of the passenger vs. the name on the certificate?

2) Can this be used like 500-miler's in the effect of direct routings? Example: If I flew JAX-IAD-ORD would 1,219 miles, but if both cleared I could get it for just the equivilant of JAX-ORD (864). Can this 1k cert. be used in the same way? or is it only for one segment/flight?

2b) To use the 'direct routing' method, does there have to be a non-stop flight between the two city pairs, or is it just two city pairs, reguardless of the flights operated inbetween them?
