Both. Once X has run out, I believe you can automatically see V class as award space and it gets converted to X if you book (i.e. it doesn't book in V so you get 25% miles back). Still, spending one's hard-flown miles in Y isn't my idea of useful award availability in general!
Would also agree with that. Back in December I had redemption booking, EDI-LHR-DTW that allowed only 75 min transfer. No alternative options showing online but the agent at the Gold desk was incredibly accommodating, even at 36 hours before departure and rebooked me onto a EDI-LHR flight for the previous evening.
Good job too as the original shuttle arrived well after boarding commenced on the connecting flight.
