UA charges $80 each way for pet-in-cabin. While expensive for somethign that costs the airline nothing, it is probably cheaper than buying an additional round-trip ticket, so I assume the other poster paid extra to have another seat simply for the dog's comfort.

As for the IDB question. I'd do a search on "Customers of Size" because we've surely had this discussion before on what happens when an extra seat is IDBed.
