Go to that link and on the page is a form you can fill out and print.

Fax copies of your receipts to the number shown on the page using the printed form as a cover page.

Oh, and you can only get credit for up to three rooms, so send the three most expensive receipts.

Hmm, what else? Make sure your name is at the top of the receipts, and that the hotel name is there too. Strangely, some hotels don't print any kind of letter head or address or anything that shows the receipt came from them.

Keep an eye on your account, and if it isn't posted, fax it in again. Or call up someone and have a strongly worded conversation that will only ruin the rep's day and your blood pressure count.
