It has been awhile since I booked only someone else (non-elite) who wasn't on my PNR or traveling on the same flight as me, but I seem to recall that I could book others into premium seats, except for exit rows. If I'm traveling with someone who's on the same PNR, I have no difficulty putting both of us in exit rows via Continental.com. Also, if I'm traveling on the same flight as someone, but we're on different PNRs, I have had no difficulty moving that person into an empty exit row seat adjoining mine by phoning the elite desk.
