They might have thought, "I will buy the cheaper ticket, and if I have to stay longer, I am sure the airline will let me, because I have medical problems.

That's a big problem. People constantly buy restricted tickets to save money, then complain because they believe they have a valid reason to avoid the restrictions. They could try CS again, but if rejected, there really isn't a whole lot left here. They might consider using an award to get back. There is no penalty for using an award ticket one way.
Actually, they booked the return flight for 10 days after her last scheduled chemo treatment. Unfortunately, she needs 6 weeks more, which is beyond the 120 days. Having cancer is just so inconvenient sometimes!

I'm trying to help out a couple of friends who have enough stress and anxiety in their lives right now.

If it can't be done, then so be it. As the Buddha says, if you can change it, why worry? If you can't change it, why worry?
