My example was when booking a ticket over the phone with BMI just over a year ago (booked a connecting flight for the v.cheap MAN-LAS in Biz I found out about through FT )
Got a paper ticket, PNR, everything was confirmed in checkmytrip.com, I even phoned to confirm my flight 24 hours before. Only when I got to the airport and tried to check in did I find out that the CC payment hadn't cleared so that I had to pay for the flight before I could board it.
