Does anybody have the link? I used to have it...lost it, looked on the website, and can't find it anywhere.

For some reason I remember it being different from thethe customer service one: http://www.usairways.com/customers/c...meraffairs.htm. The page used to have a picture of a crew inside a plane...or something like that with a form you fill out.

Haven't been able to use it in quite a while. But just had a really awesome crew on a flight this week and wanted to send off a letter. One F/A was just an angel.
