Here's what I recall off the top of my head from recent travels:

BNA - terminal B near the USAir gates has a smoking lounge. (I think A&C also have smoking lounges, but not sure).

CLT - restaurants on the concourses (Nascar, some seafood place, and a Budweiser place).

PHL - nothing unless you want to walk outside, past security (hint: Terminal F has faster security on the way back through).

SDF - smoking lounge next to the business center (IIRC, you have to go upstairs to get in)

ATL - Bunch of smoking lounges on the concourses, Sojourner's Lounge on Councourse D; also Budweiser Brewhouse

BWI - nothing.

SLC - smoking lounge down escalator from the United gates

RDU - several restaurants.
