On the 5* smaller hotels I think the Carlton Baglioni on via Senato is excellent. Smaller rooms but great service, and a private "passage" to the via della spiga (the best shopping street).

I have never stayed at the Bulgari, but had dinner there a few times (150 euros a head typically), but every friend who stayed there was very disappointed with the service, definetely a case of style over substance.
