Mileage Plus 25th Anniversary Sweepstakes Official Rules



Sweepstakes Official Rules:




NO PURCHASE IS REQUIRED TO ENTER OR WIN. A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING.


ELIGIBILITY: Sweepstakes begins on 3/27/2006 and ends 6/16/2006 (“Entry Period”). Sweepstakes is open to legal residents of the fifty (50) United States and the District of Columbia who are 18 years or older as of 3/27/2006, and are United Mileage Plus® Members (“Members”) as of 6/16/2006. Employees of United Airlines (“Sponsor”), its subsidiaries and affiliates, and its advertising, promotion, and prize agencies, including Frankel and The Ignition Network, their respective subsidiaries and affiliates, and any entity involved in the development, production, implementation, administration or fulfillment of Sweepstakes (collectively “Sweepstakes Entities”), and their immediate family members (spouse, parents, siblings, and children, regardless of where they live) and persons living in the same household as such individuals, whether related or not, are not eligible to enter or win. Void where prohibited.


HOW TO ENTER To register for the Sweepstakes, log on to www.united.com/25million. Members must first register to be eligible to receive entries. Members must register by 6/16/2006 to qualify. Once registered, you will automatically receive one entry for each qualifying segment flown on United (including United Express and Ted) during the Entry Period after registration when you book your flight with your United Mileage Plus® number. Qualifying segments must credit to your Mileage Plus account by 6/17/2006 to be eligible for entries. A segment is defined as any single non-stop flight between two destinations. For example, a non-stop flight one way from Los Angeles to New York is considered one (1) segment. A round trip flight with a return flight from New York is considered two (2) segments. If you are not a United Mileage Plus member, you must first obtain a United Mileage Plus number online at www.united.com. There is no cost or obligation to become a United Mileage Plus member.


TO ENTER WITHOUT OBLIGATION: To enter without purchase or obligation during the Entry Period, write your name, complete address, city, state, zip, daytime and evening phone numbers, your United Mileage Plus® number and the words “25 Million Miles” on a 3” X 5” card and mail to: United 25th Anniversary, P.O. Box 1774, South Bend, IN 46634-1774. If you are not a United Mileage Plus member, you must first obtain a United Mileage Plus number online at www.united.com. There is no cost or obligation to become a United Mileage Plus member. Each mail-in card will receive two (2) entries into the Sweepstakes. Enter as often as you like, however, only one (1) card per postmarked envelope will be accepted. No mechanically reproduced, illegible or incomplete entries will be accepted. Sponsor is not responsible for mutilated, illegible, incomplete, late, misdirected, postage-due, lost or stolen mail/entries. All mail-in entries must be postmarked by 6/16/2006 and received by 6/23/2006. All entries become the property of Sponsor and will not be returned.


PRIZES: Twenty five (25) Grand Prizes: One Million (1,000,000) Mileage Plus miles, awarded as a credit to the winner’s Mileage Plus account and $5,000, which may be used toward the payment of applicable taxes if the winner so chooses. Approximate Retail Value (ARV) of miles: $20,000. ARV of each prize: $25,000. Total Prize Pool ARV: $625,000. Limit one prize per United Mileage Plus® member. No transfer, substitution or cash equivalent of prize is permitted except at the sole discretion of Sponsor. Winners are responsible for all federal, state and local taxes. Each Grand Prize winner will receive a 1099 Miscellaneous Income form reflecting the value of the prize as listed above.


DRAWING: On or about 6/26/2006, a drawing will be conducted by The Ignition Network, a third party administrator and judging organization whose decisions are final in all matters, to select twenty five (25) Grand Prize winners from all eligible entries received. Odds of winning depend on the total number of eligible entries received during the Entry Period.


PRIZE CLAIMS: Each potential Grand Prize winner will be notified via traceable method of delivery. Each winner will be required to complete, sign and have notarized and Affidavit of Eligibility, Liability and, where legal, publicity release, and complete and sign a Federal Form W-9 for tax purposes, both of which must be returned within seven (7) calendar days of the notification date. Failure to return documents within the specified time frame, as well as return of any notification as undeliverable, or the inability of Sweepstake Entities to contact the potential winner, will result in forfeiture and Sponsor may select an alternate winner. If a prize is won by a person who is not of the legal age of majority in the state of residence (“minor”), the required documents must be confirmed and signed by winner’s parent or legal guardian, and the prize will be awarded in the name of his/her parent or legal guardian who will be required to fulfill all requirements imposed on winner set forth herein.


PRIZE RESTRICTIONS: Prizes do not include insurance, gratuities, incidental charges or any other personal expenses associated with use of the prize, as described herein. Prizes may not be transferred or substituted except at the sole discretion of Sponsor, which reserves the right to substitute any prize, or portion thereof, with a prize of equal or greater value, should the prize, or any portion thereof, be unavailable. Reporting and payment of all applicable taxes, fees, and/or surcharges, if any, arising out of, or resulting from, acceptance and/or use of a prize, are the sole responsibility of the winner. Use of miles is subject to the rules of the United Mileage Plus program, including, but not limited to, blackout dates, taxes, fees and restrictions. All additional taxes and fees, if any, related to travel using awarded miles are the responsibility of the passenger.


GENERAL TERMS: This Sweepstakes is governed by the laws of theUnited States and all entrants hereby submit to the jurisdiction and venue of Elk Grove, Illinois for the resolution of all claims and disputes. By participating in the Mileage Plus 25th Anniversary Sweepstakes and accepting a prize, winners agree to (a) abide by the Official Rules and the decisions of the judges and Sponsor which shall be final in all matters, and (b) to release, discharge and hold harmless Sponsor, its subsidiaries and affiliates, and its advertising, promotion, and prize agencies, including Frankel and The Ignition Network, their respective subsidiaries and affiliates, and any entity involved in the development, production, implementation, administration or fulfillment of Sweepstakes and the respective employees, directors and officers of the foregoing (collectively “Releasees”), from and against any/all claims, losses, injuries or damages, including death, resulting from the acceptance, use or misuse of any prize or any travel thereof. Except where prohibited by law, participants or, if a minor, his/her parent or legal guardian grant Sweepstakes Entities the right to use and publish their name, city, state and likeness for advertising and publicity purposes in any and all media now or hereafter known throughout the world in perpetuity without compensation, permission or notification and, upon request, will provide consent to such use in writing.


LIMITATIONS OF LIABILITY: Sponsor is not responsible for lost, late, stolen, illegible, misdirected, incomplete, mutilated, or postage due mail; any incorrect or inaccurate information caused by web site users or by telephone equipment or programming associated with or utilized in the Sweepstakes; any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access of the web site; any injury or damage to participants or to any other person’s computer(s) related to or resulting from participating in this Sweepstakes, or downloading materials from or use of the web site. Sponsor reserves the right at its sole discretion to cancel, modify or suspend this Sweepstakes, should fraud, virus, bugs or other causes beyond the control of Sponsor corrupt the administration, security or proper plan of the game. In the event of such action, Sponsor will award all prizes in a random drawing from all eligible, non-suspect entries received as of the time of such action. In the event of a dispute regarding the identity of the person submitting the entry, the entry will be deemed as submitted by the authorized and/or registered Mileage Plus Member.


WINNERS LIST: For a list of Official Winners, available after 9/30/2006, send a self-addressed, stamped envelope to: United Anniversary Winners, P.O. Box 1731, South Bend, IN 46634-1732. Requests must be received by 6/30/2006.
