ETOPS is not just an engine thing. Its a whole airplane thing. Those airplanes are "special" in that they have special radios, different maintainence procedures and different equipment.
When using an ETOPS plane for an ETOPS route, there are a lot of considerations that go into play. Dispatchers look at planned routes, fuel requirements, alternate airports and the weather.
They can use ETOPS planes on non-ETOPS routes.
