At least BA isn't currently trying to get both fees at once - I booked over the phone yesterday (open jaw transatlantic mixed classes - no choice). I got hit with the £15 fee, but paid with Amex and paid no further surcharge. The Amex cashback should almost cover the fee, so in the end I'm not too badly off.
