I purchased my tickets on all UA metal at United.com, however I was late checking-in for my flight coming home from MCO to SFO last year. The nice check-in counter lady ended up routing me through LAX on a later flight... however when I checked my MP balance a month later, I was missing the online booking bonus. I sent them an email and three days later they credited my account in the bonus column... 500 miles on the outbound leg and 500 miles on the inbound leg.
In November, I was doing a MR. My original itinerary was SFO-ORD-TPA-DEN-SFO. But in TPA, I found out that my TED flight to DEN was delayed by three hours so I'd miss my connection in DEN for my flight home. So the nice gate agent put me on a flight that would give me more miles and get me home the same night via IAD. So my new itinerary looked like this: SFO-ORD-TPA-IAD-SFO. But when I checked my MP balance a few weeks afterwards... I noticed I was missing 1,000 miles... either the online booking bonus or the West Coast offer (as this was my first qualifying trip). I emailed United and they promptly credited my MP account within 48 hours.
