My recollection is that the reason that US stopped installing them (as well as many other carriers) is simply that the use of them dropped bigtime as cell phones took off. For example, according the SeatGuru.com, both AirTran and JetBlue do not have airphones installed. If it's not being used - why bother installing new ones on aircraft as they arrive from the manufacturer.
