FWIW, I do know airlines sometimes completely go by name... was flying CO EWR-MAN once and two people went to the FA saying they each had the same seat. A cross-check of the boarding cards revealed that, yes, they did. And they had exactly the same name. But different FF numbers.

So I guess this sort of thing could happen w/ record locators?
