It is possible to mail the relevant information. My suggestion would be to call the MP hotline for your relevant elite level, speak with a supervisor, take down his/her name and the address of the service center where he/she works, and send the letter to their attention. I did that and was able to get missing flight credit from NZ applied within a week.
