I believe that airlines still do allow you to voluntarily reschedule or cancel in case of illness of the passenger or a traveling companion or a family member. I've never tried it myself, though it is buried in those lengthy terms and conditions lists, at least on UA. I'm not sure what the documentation requirements are but I don't think any change fees apply.
