Visa and MC tack on only 1%. There is a huge thread about this in MilesBuzz and a more manageable summary here.

I can personally verify that the Visa issued by my Massachusetts credit union doesn't tack on any more than the 1% Visa surcharge. And, the credit union doesn't assess me any fee for my overseas ATM transactions. More people should check out their local credit union-- big banks are increasingly for suckers.
