While the DC-9/MD-8X series jackscrew is not a "fail-safe" design, it was the lack of proper maintenance/failure to replace a worn unit of AS 261's that led to the failure of the unit and the loss of the airframe.

So provided you maintain them properly, and replace them when they near the end of their service life, they should not fail.
