OK, so what it seems is happening here is that you are getting the return LH fare without their fuel surchage. When united.com or a UA agent pulls up this fare, the automatic fare calculation adds in a YQ surcharge which is the LH fuel surcharge. I think it might be $55 for transatlantic trips and maybe more for the intra-Europe connection, or, maybe it is waived for connections to intercontiental, I can't recall.

In this case, it seems ITA is incorrect, but united.com & the UA agents are correct. I guess it had to happen sometime.

Yes. But you can't upgrade that fare anyway since it is an S fare. If you want to upgrade in advance, you have to book UA flights with UA flight numbers.
