How long have they been missing? Have you requested credit for the missing miles, and if so, via which mechanism?

I have generally had missing flights show up within 48 hours of sending the BPs via fax. Other mechanisms (the phone request, online request, and mail-in requests) seem to have little logic to when / if they show up.
