I have taken the train from Penang to Bangkok, a long 22 hour journey, but a very comfortable sleeper. You can also manage to stop at Hat Yai in southern Thailand.

I also took the train a couple times KL-Singapore overnight, again same deal, very safe and comfortable, at 40RM each way.
Book online to get prices in RM instead of SGD

However this was 3 years ago now.
