Why is an offer of miles absurd an insult? I think it is a nice way of saying "thank you, we appreciate your stepping forward to help". We had a medical on my flight once and no one came forward. Fortunately I have some medical background and handled it fairly well. Once it was determined that the pax would be o.k., 3 doctors came forward. One wanted to know how he would be compensated if he helped us.
