I have never been told no. Most of the time they remove the fee without me asking. A few times I have had to ask. Twice, with Chase, I have even paid it and they sent me a check for the return.
Not for me. Here is the e-mail:

As your credit card company, we value your business and want you to be completely satisfied with your credit card account.

Please be advised that we would be unable to refund the $65.00 annual membership fee billed to your account in the event that your account were closed. Our records indicate that, based on the activity on your account following the billing of the annual fee on July 1, 2005, the account earned reward miles.
