It almost happened once. Planned a return trip to Rio with my spouse and this time the Brother-in-law wanted to join us. We agreed and booked a larger more expensive apartment (shared cost to be paid on arrival) that could accommodate us all. Turns out the Brother-in-law left his Brazil visa to the last minute even though we had warned him. When he meets us at the airport in Rio he shares the tale of his visa woes and I ask him what would have happened had he not gotten it in time, his reply "I wouldn't have come".
