My father was on the LAX to KOA flight, is 2P, used miles and was on. I was 1P at the time and did not make it. Didn't think to check the availability before the flight. Only one person was called for an upgrade at the gate, only on one of the flights.
However, I've been waitlisted a couple times getting onto F on explus DEN-MEM and got upgraded at the gate.
Well once it went to airport control (24hrs before flight) the waitlist is dumped to DM list and resorted by status, I've noticed that it seems if a waitlist goes to the gate and its a popular route its pretty much 1Ks and GS that stand any chance. If your a lower level status and its: a) a popular route (ie. HI, transcon, etc) and b) the upgrade is important to you, check to see if you can confirm the upgrade with miles at booking to lock it in.
