Sorry to dissapoint but, the form you saw was an incident report due to the medical emergency. There is no such thing as an upgrade voucher available to the FA's.
Do the F/A's have medical incident reports onboard? If so, it would seem to me that they could have given the passenger a copy of the medical incident, which I am sure could include the information that he moved so the spouse of the affected person could be nearby. If you have that, and instructions to fax your boarding pass, reciept, and medical form to We Care with a request for compensation, you might get a nice voucher. Total speculation, but it seems that the CO people in the know all agree the voucher itself doesn't exist.
