$125 is nothing compared to the value of what I would have lost. I am eternally grateful

I don't quite understand what this means

Once you have decided to send us the card, you can call in to our service department, and you will be sent a form to fill out. Then include the completed form with your card when you ship it to us. Click here to download the form.

Can I just download the form, fill it out and send it to them?

It also keeps talking about a card
