Not got much of an answer on the first one, but as far as the rest goes - look out for things like I class fares - not too expensive and you get CE miles and tier points. Then there;s the Tesco tricks to build up a points balance, a BA Amex to get the 'two for one' voucher. And if you relly want to build up the points, and highly suitable if the flight is more fun than the holiday, head over to the Oneworld forum and read up on the xONEx deals (a Oneworld round-the-world ticke) - you can get from Blue to Gold on a single ticket.
