Actually elevator accident was not empty shaft but rather elevator stopped in between floors. A maid was onboard. Instead of waiting for help, one of the cleaning crew went to floor below and tried to help her down. They missed and she fell to bottom. That is why all elevator rescues are supposed to be to the floor above when elevator stuck between floors.
