You're right. It's there on p 59 of the guide. I cannot copy it as it is a pdf file, but it reads.....the quarter after 36 months...

I also read the FTL exemption.

I presume that means that the quarter after my wife's FTL expires, she loses all her M&M miles that are over 36 months old.
