$99 gets you the junior pack (forget the name) with 5K miles + other goodies. $119 gets the Prestige Pack, with 7.5K miles + even more goodies.

You can buy either one once a year. Call the OnePass Service Center and ask them; they should be able to sell you one over the phone.
