Getting a "Preferred Exception" questions

I've got a multi-part problem I need your help with:
I wrote CS concerning the Silver Elite offer I got from NW through Starwood to see if they would upgrade my DM status accordingly. Somehow the email went in his name and not mine (must have logged in with his #) They wrote back to him saying to fax a "Preferred Exception" request with proof of the other status. Is there a special form or do I write another request letter?
Now, Hubby and I are share and share alike. The NW offer came to me because the Starwood account has my name first. We linked the DM VISA to his DM account because he used to do more flying than he does now. His account has a much higher DM balance than mine. Should I see if NW will give him Guest Silver so the US Preferred will go to the larger balance, or can I get US to transfer the points to my account and change the VISA link?
In spite of all the changes looming, I'm planning on sticking it out with US, and picking up a little status through the back door would be nice. Thanks for helping.
