Which RTW fare carries the lowest Re-issue fee?

And you've whom have done RTW do you find yourself re-issuing tickets in the end or do you just take seperate flights?

Also doesn't really most the airlines issue E-tickets today? (If they do will there be the issue of making it a e-ticket instead of paper)
