Some also want to be use their miles to upgrade into first on international flights. You rarely can do that on the heavily discounted fares.
But there are different fares that are refundable. For example, V with United is fully refundable, so why buy an M fare? It just seems like some fares don't really serve a purpose besides being more expensive.
