I once opened the lav door and found a woman sitting on the toilet! She hadn't bothered to slide the lock to the "closed" position, and I hadn't noticed anyone heading to the lav in the previous few minutes. The green indicator light was on, suggesting the lav was vacant, as was the little sign on the door. This was in Business Class on CP.
