I was on a US flight with some of these recently and didn't have anyone wandering around, like clueless people did before they came back. Even NW's "velvet rope" was generally enough to demonstrate to people that they should not always be heading forward to get to the lav.

Interestingly enough, one could argue that these are needed for security purposes to drive more people to the rear lav/galley.
