I never had such an experience... I received my blue card about 5 weeks after my first qualifying transaction in September 2002, and that includes filing a missing mileage request.

My Elite card came really quickly though, about 1-2 weeks after the qualifying flight.

I would suggest getting in touch with them, preferably email. They are pretty prompt in replying emails. Better than being put on hold for 10 minutes on the phone.
