This accident is starting to remind me of one that happened in I think Korea in the early to mid 90s. My recollection is a little sketchy, my info came from a retired CP pilot who was an investigator (now deceased) for some pilot's assoc.
Bottom line is the 2 man flight crew were indecisive and not in agreement with what to do. One pilot wanted to abort the other wanted to continue, they argued and wrestled the controls ending in a crash, almost all survived.

You might be thinking about Korean Air 801 which crashed exactly 8 years ago today in Guam.
There were many factors contributing to that crash, but I believe the main one was the fact that the pilot in command (in this case the captain) descended prematurely without consulting the NOTAMs which would have told him that the ILS and MSAW at GUM were out of service.
