Utopia Tours provided a guide for a day tour of Ayuthaya and arranged a 3-day of Angkhor Wat in Cambodia. They aren't cheap, but the arrangements and guides were very professional. As I recall, Utopia's office is located in the lobby of the Tarntawan Hotel in Silom.
