Well, I think the issue is that they're generally NOT competing with larger, established global airlines on most of these 757 routes. In fact, they're not competing with anyone on many of them! Maybe with DL's 767 on NYC-TXL, but those seats are just TOO narrow!

Yes, I've flown on these seats. No, they're not great. Yes, they are MUCH better than coach. That's why I think they could get some premium. I was not comparing them directly to WT+ or PE, other than to say that those airlines can get a premium fare for a moderately better seat than coach with the same service.
