Last year I did the PDX-YVR-LHR-MXP route. I only had to clear customs upon arrival in MXP. I was able to go from gate to gate with no problem. The Vancouver airport is easily navigated and the connection time seems to be doable, absent any delays that is.

On the return we cleared US customs in Canada. It still confuses me, but we did it.
