I've done this a few time on CO. What I did to facilitate the process was to book me and the wife (and kids after they came along) into adjoining seats towards the back in the non-premium seats. I would then call up customer service and ask to move up all of us to a premium row. I've done this on CO a number of times - I don't know if it would work on AA, but it's worth a shot (and of course having a willing agent on the other end helps too).
