I would recommend calling your credit card company before going - I know that NJ is right in terms of Visa & Amex - Amex does not cover rentals in Italy, however, Visa does. Also, theft protection is mandatory - and if one wants their credit card to cover the CDW/Super CDW, then they need to decline the CDW or else the CC company will NOT cover it. The agents are pushy & the contracts are mostly in Italian - so make sure they go over it with you. Also, if you have your #1 Gold club activated for outside the US (which is something that you need to do seperately) make sure that you fill out the form & specificy that you don't want the CDW included.
