In my case when taking awards from my wife's account (different last name) it has always been her name on the award certificate. I call HHonors and make sure they have my name on the reservation. She has booked awards for me when they were rooms I used only for myself and my name was never on the award. I was just looking at an old reservation and it stated in the notes "Guest checking in is a diamond member." (My wife was not a diamond member.) Never have had a problem with the award certificate not having my name.
