I did a little checking. Looks like the new ID requirements may make it soon (if not already) impossible for tourists to have prepaid phones in Japan. See http://www.vodafone.jp/prepaid_en/news/keiyaku_e.html

The clerk will sell it to you, assist you in getting it going and then voila, you are ready to go. The initial charge in the battery is negligible, so I recommend buying it on the way to the hotel or the day before you intend to use it heavily so as to allow charge time.

Recharging the phone with prepaid minutes is very easy as described above and can be done at a kiosk at the convenience store on your own or with the assitance of a clerk if you would like help.
